Notary journal (Journal return)
Notary journal may be surrendered in person or by mail.
In-person
Make sure that the notary public is commissioned for Santa Clara County.
The following information is needed when a notary public surrenders their notary journal:
- Notary name
- Notary address and phone number
- Notary commission number
- Expiration date of commission
- Indicate the number of journals surrendered
- Indicate the Bond Company name
By mail
The notary journal must be mailed by certified mail with return receipt to:

- Provide a self-addressed stamped envelope so we can return the Notary Journal Receipt form.
